NOMINATION FORM:  *VOSH/INTERNATIONAL  HUMANITARIAN OF THE YEAR 2013
VOSH/International presents the HUMANITARIAN OF THE YEAR AWARD at the annual meeting to honor an individual who has distinguished him or herself by bringing eye care to underserved populations.  The award recognizes a VOSH member who has improved visual welfare through leadership, invention, public health, developing sustainable eye care and furthering the understanding and correction of refractive error .  To nominate an individual please complete and return the attached application form by May 1, 2013.  The VOSH/International Humanitarian of the Year 2013 will be decided on or about June 1, 2013 and presented at the V/I Annual Meeting in October 2013.   Submissions may be sent electronically to  < This e-mail address is being protected from spambots. The VOSH/INTERNATIONAL  LIFETIME  ACHEIVEMENT AWARD is given at the discretion of the V/I Board to a person who has been recognized during a lifetime of service to humanity for improving visual welfare through leadership, invention, public health, developing sustainable eye care and furthering the understanding and correction of refractive error. It is not an annual award and meets the highest criteria that exemplifies the spirit of VOSH/International.
